Blank Registration Card
Print the above registration form, completely fill it out, fold it in half, staple or scotch tape (close) it, put a stamp on it, and drop it in any U.S. Postal mailbox. When the Registration Card is printed it shows the front of the card above the back side of the normal card. The front – back measures 4.25 inches height by 5.25 inches width. Since both the front and back of the Registration Card are included the printout, adjacent, on the same page, printing the file results in 8.5 inches height by 5.25 inches width (this fits well on a standard 11 inch height by 8.5 inch width paper size).
